Michigan Long-Term Care Connection Logo The Michigan Long-Term Care Connection (MLTCC) is a coordinated system of information and assistance in planning and arranging for long-term care services. Access to service is simplified and streamlined.

The Long-Term Care Connection:

  • provide comprehensive Information and Assistance services and Options Counseling to help consumers learn about, understand and explore options, as well as to plan for and access long term care services and supports.
  • facilitate the person-centered planning process with consumers.
  • conduct Functional Eligibility Determination and coordinate Medical Financial Eligibility Determination;
  • offer Benefits Counseling to help people learn about and apply for public and private benefits;
  • facilitate information about nursing facility transitions and assist in the development of a transition plan; and
  • serves as Michigan's single point of entry into Medicaid-funded long-term care services;

Information specialists are available at each LTCC site to provide unbiased information about programs and services so that you and your family can make an informed choice. Through a person-centered planning process, options counselors assist consumers to set goals, make choices, and plan services in keeping with their own individual strengths, needs and resources.

Serving residents of the cities of Detroit, Grosse Point (GP), GP Farms, GP Park, GP Shores, GP Woods, Hamtramck, Harper Woods and Highland Park

Southwest Michigan

Serving residents of Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, St. Joseph and Van Buren counties

West Michigan

Serving residents of Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Muskegon, Newaygo, Oceana, Osceola and Ottawa counties

Upper Peninsula

Serving residents of Alger, Baraga, Chippewa, Delta, Dickinson, Gogebic, Houghton, Iron, Keweenaw, Luce, Mackinac, Marquette, Menominee, Ontonagon and Schoolcraft counties
